Minocycline Topical Foam 1.5% Approved by FDA for Rosacea

FDA approves first topical minocycline Foam 1.5% ZILXI for the treatment of inflammatory rosacea. Minocyclin is a tetracycline-class drug that was originally approved as an oral formulation for the treatment of acne vulgaris but now being developed into topical formulation to treat both acne vulgaris and rosacea. Previously on Oct 2019 same formulation in the name of Amzeeq 4% topical foam was approved for the treatment of acne.

ZILXI, developed as FMX103 by Menlo’s wholly-owned subsidiary Foamix Pharmaceuticals Ltd. (“Foamix”), is the first minocycline product of any kind to be approved by the FDA for use in rosacea.

“This approval is welcome news for clinicians and patients who seek novel options for this difficult to treat skin disorder,” said David Domzalski, Chief Executive Officer of Menlo. “ Minocycline Foam ZILXI 1.5% is a potential turning point in rosacea treatment, providing millions of people with a new treatment option that is well-tolerated and effective.”

Rosacea is a diverse skin condition that most commonly presents with symptoms such as deep facial redness, spider veins (telangiectasia) and acne-like inflammatory lesions (papules and pustules).

Minocycline is one of several broad-spectrum antibiotics known as tetracyclines with anti-inflammatory properties; their use in some patients is limited due to systemic side effects when taken orally. In ZILXI, Menlo has once more leveraged its proprietary Molecule Stabilizing Technology (MST™) platform to effectively deliver minocycline in a foam-based vehicle.

“This is the only product containing minocycline approved by the FDA for rosacea,” said Iain Stuart, PhD, Chief Scientific Officer of Menlo. “The availability of a novel topical formulation of this molecule underscores our efforts to provide innovative treatment options for patients who suffer from difficult to treat skin conditions.”

The FDA approval of ZILXI is primarily supported by data from two clinical trials in 1,522 patients 18 years of age and older. In each 12-week multicenter, randomized, double-blind, vehicle-controlled trial, subjects with inflammatory lesions of rosacea were treated once daily with ZILXI or vehicle. No other topical or systemic medication affecting the course of inflammatory lesions of rosacea was permitted for use during these trials.

The co-primary efficacy endpoints were (a) the absolute change from baseline in inflammatory lesion counts at Week 12 and (b) the proportion of subjects with treatment success at Week 12 defined as an IGA score of 0 (“clear”) or 1 (“almost clear”), and at least a two-grade improvement (decrease) from baseline at Week 12. ZILXI met both co-primary endpoints in each clinical trial, demonstrating statistically significant improvements in inflammatory lesion count and Investigator Global Assessment (IGA) treatment success.

No treatment-related serious adverse events were reported. The most common adverse reaction reported by ?1% of subjects treated with ZILXI and more frequently than in subjects treated with vehicle was diarrhea (1% vs. 0%), respectively.

Menlo anticipates having ZILXI available for prescribing by 4th quarter of this year.

10 Tips to Prevent masacne when wearing masks

  1. If you are wearing a cloth mask, wash it every day. If a disposable one, discard it when it becomes damp or at least air it out.  Apply silicone gel strip in the pressure point if you are wearing N95 mask for long.
  2. Don’t touch your protective mask often; always wash your hands after you touch on something.
  3. Wear well fitting mask, do not use tight masks.
  4. Cleanse and moisturize your skin immediately after you remove your mask. Be gentle, use gel or water based moisturizers that can help with clogged pores. Moisturizers also hydrate the skin, rebuild the barrier and give it a chance to heal quickly.
  5. Wash your face at least twice daily with gentle cleanser. For acne skin you can use mild medicated cleanser containing salicylic acid or benzyl peroxide to effectively remove oil and to provide an antibacterial and anti-inflammatory effect.
  6. Remember not to over wash your face, when you are over washing you are stripping natural oils off your skin and making it more irritated.
  7. Avoid the use of makeup, foundations and concealers which leads to clogged pores resulting in acne breakouts.
  8. When frequents washing is impossible you can use oil-free wipes to freshen up the skin.
  9. If you have active acne, treat it as quick possible to avoid further complications. Your doctor will suggest you anti-acne medications that best suits your skin. Don’t try to treat it yourself with strong medications like adapalene which can make skin more irritating.

Antibiotics for Acne

Antibiotic therapy (topical or oral) is commonly used to treat acne in many patients and is usually considered to be safe and effective. The mechanism by which these antibiotics for acne work differs with the drug. Some antibiotics have antibacterial properties with minimal anti-inflammatory effect, while some have both anti-inflammatory and antibacterial properties. Percutaneous Collagen Induction for Acne Scars

Today, many people wants a fairly rapid solution to treat acne scars. These has brought many different topical scar removal medications and treatment techniques like laser resurfacing, dermabrasion, carbondioxide lasers, deep peelings and many others which are quite effective, but the side effects and chances of scarring is still possible, especially for dark skinned peoples. So to avoid these side effects while effectively reducing acne scars, percutaneous collagen induction therapy was developed.

Percutaneous collagen induction therapy or medical needling is a technique where drum-shaped roller with fine microneedles are used to puncture the skin and release the growth factors which encourage the formation of collagen and elastin and further helps in reduction of acne scars. Recent studies suggest that combination of  Percutaneous Collagen Induction Therapy ( PCT) and Photo Dynamic Therapy ( PDT ) has shown better results then PCT or PDT alone. Laser Treatment For Acne Scars

Lasers Treatment are now very popular treatment of different skin conditions including acne. They use different wavelength and intensity and remove the scar tissue and reduce the inflammation and redness of skin around healed acne lesions. Various Laser treatments are available and the choice of laser treatment depends on type of Acne scar.

There are 2 main categories of laser treatment used in acne scar therapy which are Ablative lasers and Nonablative lasers. However, these laser treatment may only treat the superficial skin layers and is not much effective for deeper scars like ice pick scars.

Ablative Lasers:
Ablative laser are mainly used to treat on the surface skin cells( Epidermis). These Lasers emit high energy densities at extremely short pulses to heat and vaporize target tissue with limited damage to surrounding tissue or skin. They offer better control of resurfacing compared with dermabrasion. Ablative Lasers use either carbon dioxide or Er:YAG. The Er:YAG laser is a more gentle ablative therapy than the carbon-dioxide laser. This may be of benefit for hypertrophic scars, rarely keloids, and shallower boxcar scars. For other icepick scars, Rolling scars and sallow boxcar scar dual-pulsed modes or dual-mode Er:YAG may be beneficial. The healing process may take upto 6-8 weeks and if needed scar is treated with additional laser sessions.

Although these lasers have been considered as “gold” standards for skin resurfacing there are more potential side effects, which may include delayed healing, erythema, milia, acne, edema, hyperpigmentation or hypopigmentation, infection, or even scarring . The risk of post laser scarring is even higher with CO2 laser resurfacing compared with Er:YAG resurfacing because the thermal tissue damage is more with CO2 lasers then Er:YAG lasers.

Non-Ablative Lasers:
Non-ablative lasers are solely used to treat below the skin usually on mid layer of skin ( Dermal Collagen) . These lasers may include 1320-nm or 1064-nm neodymium:yttrium-aluminum-garnet lasers (Nd:YAG) and diode (1450 nm) lasers, and the 585-nm pulsed dye laser (PDL). Side effects are generally mild and the effectiveness of reducing acne scarring may also be lower then ablative lasers. It has been suggested that best results may be achieved when these lasers are combined with another modality such as surgery or chemical peels. Because these modalities are less aggressive as a whole, they are more useful for atrophic, rolling, or possibly hypertrophic scars rather than icepick, boxcar, or keloid scars.

Although the recently developed technique of fractional laser skin resurfacing has been reported to be effective in acne scarring, the risk of hyperpigmentation/hypopigmentation, scabbing, milia etc.. may be the factors nonablative scar resurfacing are still popular for acne scar reduction.

Fractional laser treatment:

Fractional laser treatment for acne scars are now being widely popular as these type of lasers treat both outer and inner layer of the skin. But, the procedure may be painful, so topical anesthetics are used along with forced air cooling to increase patient comfort. A variety of ablative fractional resurfacing lasers are now available and the initial impression is that these lasers may be more effective than nonablative fractional lasers for acne scars. However, side effects may also be more of a concern with the ablative fractional lasers treatment. There are similar side-effect concerns as other ablatives but there tend to be less problems overall because of the selective sparing of skin rather than total ablation, there is still the possibility for transient erythema or edema, dryness, scabbing, milia or acne, hyperpigmentation or hypopigmentation, prolonged healing, or infection.

Unlike other, you can apply makeups or sunscreen immediately after a treatment session. As postoperative edema and erythema may be common, so to avoid it application of ice during the first 24 hours may be beneficial, erythema will resolves in 5-6 days.
